The OFPA 109th Annual convention will be held on Tuesday, April 11 and Wednesday, April 12, 2017 at the Holiday Inn Convention Center and Hotel,
After the form is submitted, an itemized invoice will sent via email. This form may be printed. Mail your check to the above address with your printed form or
invoice. Payment can also be made by credit card from our website.
Preregistration is $125. Deadline for preregistration is March 25, 2017. Registration after March 25 is $150.

Please indicate tickets needed for each person. The registration fee covers all convention events and the breakfast, lunch and reception. Tickets for meals are required for each person. Submit multiple registration forms if needed.
Registration and Cancellation Policy: Registration is required for all events. Registration can be cancelled by written request to
OFPA. No refunds after March 25. Substitutions are allowed. “No shows” will not receive a refund.
